I'd argue if it's completely consensual and consent can be withdrawn freely at any time then it's not a hierarchy. So (most) femdom would not be a hierarchy by this definition.

if said hierarchy has no power structure enforcing it, is isn't a hierarchy. if it's a hierarchy of trust, and respect (not sure if that's the right term, not sure if there's one) then that's likely the only valid form of hierarchy.
