Medication is mostly trial and error. What works well for you may not work well for somebody else and vice versa. You're not likely to get on too high a dose initially since they're going to want to start at a low dosage and increase as needed. You're also not going to get everything resolved at your first appointment. It doesn't take long to get results but it does take time to get everything adjusted.
Stimulants are a typical first line of treatment for ADHD. They do the job well, for most people, once you get everything dialed in, which can take a few months in some cases. Avoid caffeine, at least when you're starting out unless you like "overwhelming feelings of impending doom."

It sounds like patience amd long term persistence is needed....which casts doubt on viability but if others can do it I suppose maybe I can too
Yes, but maybe not to the degree that you're thinking. If you're put on a stimulant, which is usually a variant of either amphetamines or methylphenidate, you're going to notice a difference immediately. Ok, maybe not immediately but within 1-2 hours after taking it. Depending on how your body respond to it, you may have to change dosages or try a different variant.
Either way, you'll probably know very quickly if it's going to be effective.
