I don't get it
This is what you need to start the process of transitioning medically in some states. Particularly top or bottom surgery.
Many trans people play fallout new Vegas to the point it's a vommon joke. The joke here is that you must go through all the hoops of bottom surgery before playing the game as if it's a similar action to recieving bottom surgery
