Let's say I, a middle aged cis man went to my doctor, wanting gender affirming care, and my doctor wanted to provide - what, if anything, would the treatment/care consist of? Just making sure my testosterone level was up to Hegseth-approved standards?

That's the neat part, it is all just "care".

The whole thing gets muddy AF when one tries to squeeze definitions that make sense out of a box stuffed with binary gender and stereotyped gender "norms" that are not representative of real life. It is ridiculous to try to shove all possible characteristics of a person's body into either "masculine" or "feminine" columns seemingly just to keep score for how "manly" or "feminine" the person is. (not saying you are doing this btw, this is a critique of a lot of our current society at large)
