I've seen something like this before, but it was with the Doctor.

Doctor: "Captain! I wonder if you would like to sign this 'Get Better Soon' card for Mr. Paris."

Janeway: "'Get Better Soon?' I didn't know you were treating Tom for an illness."

Doctor: "Oh, I'm not. I simply think that he should get better. At everything. Particularly at being a medic. And that he should do so soon."
